2 things I gained in NY Pre-clerkship program

I realized the difference in medical situations between Japan and the United States. The first thing I felt when visiting a hospital or a clinic for shadowing is that doctors had high expertise and their role were fairly subdivided. When I visited a clinic specializing in fetal ultrasound, detailed ultrasonic examinations, genetic examinations and genetic counseling which cannot be accepted at general obstetric hospitals were performed. I received the impression that the division of labor system is established as a whole. This is also true in the research field. There wasn`t much equipment in the laboratory and much work was entrusted to other specialized agencies. I felt that this system is efficient for raising the expertise of each area and it is one of the reasons that the medicine of America is advancing.

日本とアメリカの医療の違いを実際に感じられたことです。シャドーイングで病院やクリニックを訪問してまず感じたことは、個々の医師の専門性が非常に高く医療がかなり細分化されているということです。シャドーイングで訪問した胎児超音波を専門とするクリニックでは、他院で何らかの遺伝子異常の可能性が指摘された場合に一般産科病院では受けられない精密な超音波検査や遺伝子検査、遺伝カウンセリングを受けることができます。それぞれの役割分担が細かくはっきりしており、分業体制がシステムとして成り立っているという印象を受けました。このことは研究分野でもいえることであり、研究室には最低限の機器しかなく多くの作業は他の専門機関に委託しているとのことでした。こうした体制は各領域の専門性を上げるには効率的であり、アメリカが医療の最先端を行く理由の一つなのだと感じました。
