Simulation Center Visit（病院研修）

The most impressive training for me was this medical examination by interview with standardized patients (SP). I heard American medical students practice interview with SP in 3rd grade but we don’t have much opportunity to interview in Japan. I think there are a little distance between my medical study and interview. I didn’t care what to ask to patients when I studied diseases. With a view to interview, my study hours would become more meaningful time. 　Through trainings at Pace University and simulation center, I learned that showing our curiosity and trying to know about patients with nonverbal communications were so important. It seems a matter of course but in Japan we apt to forgot it because we can communicate in language smoothly. And I found that there were some educational differences between America and Japan. Through these training, I felt that American teachers regarded feedback as so important. When we were talking or playing a role, they said nothing, so we could do freely. They recorded everything, and just after our time, they gave us so much feedback. Usually we don’t get such feedback in Japan but I thought this system was so effective and useful for me to understand my bad points.
和訳 今回私が最も楽しかったトレーニングが、模擬患者との医療面接です。アメリカの医学生は３年生の段階で模擬患者とこのようなトレーニングをすると聞きましたが、日本ではこのような機会はほとんどありません。今回このトレーニングを行い、普段の自分の医学知識の勉強が医療面接とあまり関連づいていないことに気づきました。これまで疾患の知識は暗記していても、何を患者さんに最も聞くべきなのか、などは気にしていませんでした。医療面接を常にイメージしておくことで、暗記の学習の時間がより効率的なものになるのではないかと感じました。 　ペース大学とシミュレーションセンターでのトレーニングを通して、患者さんへの関心を表現し、また患者さんのことをより知ろうとする上での非言語的コミュニケーションの重要性を知ることができました。一見医師として当たり前のことのように思えるのですが、日本にいる限り言語での意思疎通が簡単にできてしまうため、非言語のコミュニケーションの重要性は忘れがちだと思います。また、教育の面でもアメリカと日本では少し違うなと思う点がありました。トレーニング中、アメリカの先生はフィードバックを非常に重要視している印象をうけました。私達が話し合ったり、医師患者役を演じたりしているときには先生は何も言わず、自由に何でもさせてもらえましたが、先生は全てを記録していて、私達の発表が終わるや否や本当にたくさんのフィードバックをくれました。これまで日本の教育でこれほどのフィードバックをしかも自分の発表の直後に受けた経験は無かったのですが、自分の良かった点、悪かった点を理解するのに非常に効果的で役に立つシステムだと感じました。

At the first day of training, we felt nervous, and had no confidence, because it was hard for us to just talk with native speakers, much less to interview their symptoms. However, the more training we do, the more exciting and fun it became. Laurie, Tony, and Philip were our teachers. Their lectures were very easy to understand and improved our medical interview skills.
和訳 ネイティブの方と普通に話すだけでも難しいのに、その上彼らに医療面接を行うことはかなり困難に感じていたため、初日は緊張していて、自信も全くありませんでした。しかし、トレーニングを重ねるごとに段々と楽しく感じるようになってきました。講師をしてくださったローリーとトニーとフィリップのレクチャーはとてもわかりやすく、私達の医療面接の技術を向上させてくださいました。

This was the best training that we took in this trip. We actually took a history from standardized patients (SPs). At first I was very nervous because I had little confidence that I would do well. Just like I thought, when I began to talk with SP, I got very upset and I did not know what to say even though I had studied at Pace University a few days before. But after I finished a medical examination, SPs gave me feedbacks from a patient point of view, for example, how they felt or what I should have done. Thanks to this feedback, I could understand how important to ask a patient his/her lifestyles, family, and so on, because I can know how he/she is suffering. And I could gradually perform a medical examination more smoothly, and I got more confidence. Finally I was able to enjoy it. This was a great change for me, so I was happy to take this class.
和訳 これは私たちが今回の研修で受けた中で最も良いトレーニングでした。私たちは模擬患者の方々に英語で問診を行いました。しかし最初のうちはうまくできるか自信がなくとても緊張していました。思った通り、数日前にPace大学で練習したにも関わらず、模擬患者の方に話し始めると私はとても混乱し、頭がパニックになってしまいました。しかし一旦問診が終わると模擬患者の方が患者目線で、「どう自分が感じたか」、「私がどうした方がよかったか」といったようなフィードバックを毎回必ずしてくださりました。このフィードバックのおかげで私は患者のライフスタイルや家庭環境などについて聞くことの重要性を理解することができました。なぜならそういったことを聞くことで患者がどれほど苦しんでいるか分かるからです。また、練習を重ねるごとに次第にスムーズに問診を行うことができるようになり、より自信が持てるようになっていきました。最終的には英語での問診を楽しめるようになりました。これは私にとって大きな変化で、このトレーニングを受けることができて本当によかったと感じました。

Yuya Eguchi
We experienced more realistic simulation at simulation center. We could talk with SPs. They are actors. For medical students such as us and residents, they perform various characteristic patients. We took medical history to SPs for 15 minutes, and were evaluated. We tried this simulation many times.
和訳 シミレーションセンターでは、よりリアルな状況を体験しました。SPと話すことができました。SPとは役者さんのことです。彼らは私たちのような医学生のために、様々な性格の患者を演じてくれるのです。私たちは約１５分医療面接を行い、評価されました。このような模擬面接を何度も挑戦させてもらいました。
